Serious Case Review

The Board will conduct a Serious Case Review following the death of a child or young person when abuse and/or neglect are believed to have contributed to their death. The Board will also consider whether a Serious Case Review should be convened in cases where there has been life-threatening injury to a child, or there are other circumstances which merit such a review. The Board will advise member agencies of the outcome of the review and will indicate the lessons that are to be learned from the results of the enquiry. The Board will agree a joint action plan so that policy and practice can be improved in order to prevent any similar event occurring.
